FOOD SANITATION PROGRAM: LICENSE FEES:
FULLTIME ESTABLISHMENTS: Licenses for fulltime establishments are valid for one year from the date of issuance with license fees and application for renewal due thirty (30) days prior to expiration of the current license. Fees, which are based on potential risk for causing foodborne illness, are as follows:
Category I Food Establishment	$ 75.00/year
Category II Food Establishment	$ 50.00/year
Category III Food Establishment	$ 25.00/year
License applications that are received late will be assessed a penalty as follows:
Application for renewal received after July 31, but before August 31	$ 10.00
Application for renewal received after August 31, but before September 30	$ 25.00
TEMPORAY FOOD ESTABLISHMENTS:
Three consecutive days at same location	$ 10.00
Four (4) to ten (10) consecutive days at same location	$ 15.00
Eleven (11) to fourteen (14) consecutive days at same location	$ 25.00
Organizations having a current and valid Putnam Co. Food Service License	No Charge
