Bureau & Putnam County Health Department Food Class Schedule

15-Hour Food Service Sanitation Managers Certificate Class

When: October 23rd, 24th and 30th, 2012
Time: 9:00 am to 3:00 pm
Fee: $100 (Includes Book)

Requirements:

• Students must attend all 15 hours of the course to sit for the exam.
• A photo ID is required on the day of the exam (October 30th)
• Class size is limited to 35 students.
• Pre-registration form and fee is required to reserve a place in the class.

5-Hour Refresher Class

When: Wednesday, April 11th, 2012
Time: 10:30 am to 3:30 pm
Fee: $55

Requirements:

• Students must attend all 5 hours of the class to renew their FSSM certificate.
• The Refresher Class must be taken before your current certificate expires. If your certificate is expired you will not be allowed to take this class.
• A photo ID and copy of your current Food Service Sanitation Managers Certificate is required the day of the class.
• Class size is limited to 35 students.
• Pre-registration form and fee is required to reserve a place in the class.
