Dental Services

The dental clinic, located within the Bureau County Health Department, provides a variety of dental services to individuals from Bureau and Putnam County.

Eligible dental clients include:

Residents of Bureau and Putnam County age 3 and older with:
1. Current Illinois Public Aid Card
2. At or below 185% poverty level
  1. Services offered at a reduced fee
  2. Income verification calculated from last year's
    income tax return.

*Note that individuals with current dental insurance coverage other than the Illinois Public Aid card are not eligible for dental services.

- Check up
- Cleanings
- Sealants
Pain management:
- Extractions
- Some Root canals
Restorative Treatment:
- Esthetic (white) fillings
- Crowns
- Partials
- Smile enhancement

A reminder call will be made to you a day or two before your appointment
Bring your current Public aid card or income verification forms with you to your appointment. (Current monthly public aid cards must be brought to every visit.)
Written parental consent is required for all children (under 18 years of age) receiving treatment.
